Relational Connection

Once I receive and review your support plan and financial contribution, I will initiate e-signing our Relational Connection through LibreSign. You will receive an email requesting your signature, so look out for that.

Relational Connection

revised 3/20/26

Logistical considerations for partnership

A few other logistical things that WE BOTH DO to tend to the foundation of the supervisory and consultation relationship:

  • Maintain a record for each session.
  • Inform each other of grievances or actions that were taken against them so that we are informed about concerns within the community about the quality of our work.
  • Maintain professional liability insurance (and general liability insurance, if necessary).
  • Meet regularly as collaboratively determined.
  • Mitigate dual or multiple relationships with each other (such as friendships, business or financial relationships, social gatherings). If a dual relationship does occur, we will discuss how potential conflict can be mitigated and addressed. No romantic and/or sexual relationship will occur between us.
  • If self-employed: develop, implement, and review your practices for documentation.
  • If employed by an agency: adhere to agency standards for documentation.
  • Ensure financial contribution is collected timely and accurately.
  • Renegotiate and challenge these commitments as needed.

By signing below, we affirm the following:

  • I have completed Parts 1 and 2 of the relational process.
  • We commit to honesty and directness in our actions and words towards each other.
  • We will maintain our personal and professional integrity through the standards of practice and ethical conduct, which may exceed the expectations of DORA and our professional Code of Ethics.
  • We acknowledge that we can end our partnership at any time by informing each other that we would like to end.
After signing, please return to this page to complete the final step of the process.
